I wish to participate in indoor trampoline activities including, but not limited to trampolining, trampoline park access, trampoline dodgeball, café access, (collectively hereinafter called “the Activity”) organised by Jump Boxx Indoor Trampoline Parks, trading as Jump Boxx.

I declare that I am the parent or legal guardian of the child or I have authority from the child’s parent or legal guardian to sign this Waiver Form.

I agree that the child will purchase and wear Jump-Boxx socks in accordance with the safety instructions that will
Be given before undertaking the Activity.

I agree that the child will wear any safety equipment provided by Jump Boxx in accordance with the specific safety instructions and that he/she will follow the directives of Jump Boxx, its management and staff at all times.

I confirm that I am in good physical condition and have no medical impairment that might prevent me from my intended use of Jump Boxx.

I acknowledge that Jump Boxx did not give me medical advice relating to my physical condition and ability to use the facilities.

I acknowledge that the Activity is potentially dangerous and that by participating in the Activity I am exposing myself to the possibility of personal injury.

I accept the risk of personal injury and/or property damage I am exposed to whilst participating in the Activity.

I hereby indemnify Jump Boxx against all claims made by any other person against Jump Boxx in respect of any injury, loss or damage arising out of or in connection with my failure to comply with the safety instructions and /or directions of Jump Boxx, its management or staff.

Jumping Jaxx Soft Play

The Jump Boxx Team keep a watchful eye on children, however, children remain the full responsibility of the parent/guardian.  For everyone’s safety and enjoyment, we ask all parents/guardians to supervise their children and encourage safe play and good behavior.  Any behavior deemed to be unsuitable towards other users or staff will not be tolerated and you may be asked to leave the play area.

• Only children who are under 5yrs & below 120cm are allowed to play in Jumping Jaxx.
• All children must be accompanied by an adult over 18yrs.
• Jump Boxx socks must be worn by users at all times. Parents/guardians must also wear socks.
• Children should use the toilet and wash their hands before playing.
• Long sleeve tops and long trousers are best for play.
• Leave badges and jewelry with parents, spectacles should only be worn if used with a retainer and shatter proof lenses.
• Only 1 child per trampoline. No flips or tricks. Always land on two feet.
• Use the slides sitting down, feet first.
• Report all accidents to a member of staff immediately.

• If you are unwell please do not enter the play area.
• Clothes with cords and ropes should not be worn.
• No shoes or bare feet.
• No sharp objects.
• No food, drinks, sweets or chewing gum.
• Please do not climb the slides or play in front of slide exits.
• It is forbidden to climb on the netted walls or system structure.
• No bags, car seats or strollers are allowed inside the soft play area.
